OP, that will happen if you click play, then click again anywhere on the Fallout 4 start menu before the game launches. Only click play [once] on the menu and wait for the game to launch. Go to settings, system, console and updates, reset console and choose to keep everything. After that your console will restart and make you go through a quick set-up and sign in again.
